The difference between 老奶奶 (lǎo nǎinai) and 奶奶 (nǎinai) is that 老奶奶 (lǎo nǎinai) implies a greater age than 奶奶 (nǎinai). 老奶奶 (lǎo nǎinai) is used to refer to an elderly woman, while 奶奶 (nǎinai) can be used to refer to any woman of an older generation, or more specifically somebody's grandmother.
