Why it means “how fast he is running” not means “how fast he runs”?? Some one can explain for me?

Hi both are possible. It depends on the context. If you are looking at the person running and commenting "他跑得多快啊！" then you mean "How fast he is running!". If you are simply saying he generally runs fast, you can still use "他跑得多快啊！".
